Instructions
  • Provide complete and accurate information so work can begin on your case. There may be a delay if the information you provide is not complete.
  • A separate application is needed for each person you are seeking support from or are paying support to.
  • Copies of all orders and payment records must be provided along with your application. You will have an option to upload these documents with your application.
  • If you are a parent who receives support and you are receiving help from a public assistance program (TANF, Foster Care), your information has been automatically referred to Child Support. You do not need to complete an application for services.
  • Unfinished applications are saved for 60 days. You can return and complete your application at any time.
  • Contact the CSSD if you have any questions.
Cost of Service
The Child Support Services Division is required to charge a $25 application fee to individuals applying for child support services. No fee is charged if you are receiving Medicaid benefits. The fee is non-refundable, even if the CSSD determines your case is unworkable.
Release of Information
Information (including Social Security Numbers, names and addresses) provided in this application or through other means may become part of the public record and may be shared with others. If you are concerned that the release of case information could result in physical or emotional harm to you or your family, or if you have a protective or restraining order against a receiver of the information, you must notify the CSSD.
